RFU Safeguarding Incident Report Form

This is the Incident Report Form that should be submitted to the RFU through the Safeguarding Officer following a young person expressing concerns about someone's behaviour towards them. Advice and guidance should be sought from the Club Safeguarding Officer immediately.

This is the Junior Photographic Permission Form. Please download here and print off a copy. Once you have completed it then pass to the Safeguarding Officer. Please note team photographs or match day photographs will not be taken unless all children's parents within the team/age group have submitted a valid permission Form.
